rocker arm troble

my friend is having trouble with his self allining roller rockers he installed them and the ridges on the roller tip got bent out to where they no longer roll on 3 of the rockers what causes this

sounds as if they were not adjusted properly and got into a bind. better off replacing them with NSA rockers and add 7/16 studs along with guide plates

The alignment tabs bent towards the roller tip?
