You've either had one or will have one. I just had one a couple days back. Was fixing lights on our new (to us) 12ft pre-lit tree.

Long story short, I was standing on said chair just fine. When I went to get down, I was standing at the front edge of the seat and using my arm put some pressure on the top of the back rest. Too much outward pressure at either end and the backs of the legs split out and it was something out of a cartoon, the dang thing just crumbled under me. I got LUCKY as I narrowly missed coming down right on a leg remnant with a sharp tip pointing straight up still (have the scratch on my side and hole in my shirt as proof). Haven't bought a lottery ticket yet, probably should.

So PSA......don't be lazy like me and get the ladder out of the garage that's 15 feet or so away next time.

When I was getting off the roof I had my belt catch the gutter and almost lost the footing on my ladder.

Lesson there is always use a extension ladder, not the short of too short folding one.
