Pull the plugs and look for a shiny porcelain, will cause your popping at upper rpms.
I probably can't describe it correctly but I think its a rich condition and higher temps melts the extra gas...

Pressure. 100 is atmosphere, 90 is I think 2-3 inches vacuum. Over 100 is psi, lower is more vacuum so as you go wot you're up around 90-100

shiny glaze on the plugs will do it. gets glazed by burning a little rich then that glaze acts as a conductor or something like that. check em out.

with the scan tool, look in the datastream where it shows you what the pcm is seeing while the engine is running.

imo, do adjust the throttle blades to get iac counts in line first on a warmed up car, then adjust tps.

I know you said you have spark but is it good and strong or just there? Had a car in the shop yesterday with spark but it was weak and wouldn't run. Coil tested bad. Just a thought.

You're right, I guess it is possible some states don't look at all the emissions. I know in NC they do but now I'm curious. So what state are you in?

Torn boot is gonna let in un-metered air. The maf has to know how much air is going in the engine so the pcm can supply the right amount of fuel. Extra air coming in that the maf isnt measuring...
